    Summary

    HHAP provides grants and loans to acquire, construct and rehabilitate housing for persons who are or would otherwise be homeless. Projects eligible for HHAP/MRT funds must provide permanent supportive housing to homeless single individuals whom exhibit conditions or histories recognized to be associated with high Medicaid usage. Examples include, but are not limited to single adults who have experienced chronic homelessness, physical disability, mental illness, substance abuse, and/or other disabling conditions.

    Additional Information

    Successful applicants and their service providers will be expected to collect and submit relevant Medicaid patient data to the State Department of Health (DOH) or other state agency to track Medicaid cost savings. Since this data is classified as confidential it must be transmitted in a secure format. Additional stipulations and reporting may be required, if determined by the MRT and/or DOH.

  • Eligibility Details

    Not-for-profit corporations and their subsidiaries, charitable organizations, municipalities, and public housing authorities are eligible to apply for HHAP/MRT funding.

    Deadline Details

    The HHAP program is an open-ended RFP, meaning proposals will be accepted for consideration on a continuous basis until it is determined funds are no longer available for award or this methodology no longer serves a purpose for HHAC. This program will begin accepting applications on June 30, 2014.

    Award Details

    Award amounts vary. Funding will be made available and awards announced subject to the approval of the Commissioner of OTDA and the Homeless Housing and Assistance Corporation’s Board of Directors.
